Documents Required
- Aadhaar card of the deceased (if available)
- Aadhaar card of the applicant
- Hospital death report / medical certificate (if death occurred in hospital)
- Proof of residence (ration card / voter ID)
If hospital death report is not available: local body may ask for an affidavit, police inquest report (if unnatural death), and witness statements.
Validity
Death certificate is valid for lifetime (used for legal/settlement purposes).
Procedure
- Open the KSmart portal (link above) and go to "Death Certificate".
- Fill deceased and applicant details and upload documents.
- Submit the application online and take note of the reference number.
- Local body will verify documents; collect the printed certificate or download once approved.
